About Us

Healthy Transitions Colorado (HTC) began formally in the Summer of 2013. HTC is a coalition that seeks to connect and align the efforts of multiple stakeholders in a way that promotes cohesion to a set of common objectives and measures progress in an agreed-upon way. In addition to our 5 Leadership Partners,  HTC has over 60 pledged community partners.

The Center for Improving Value in Health Care facilitates partner and community meetings, coordinates ongoing communications, provides administrative support, and provides data to track progress from Colorado's All Payer Claims Database.

The Colorado Health Care Association provides education and policy support to its membership of over 250 skilled nursing and assisted living centers in Colorado. CHCA believes that the health care system has a responsibility to meet the physical and psychosocial needs of the Long Term Care patient in the most cost-effective manner.

 

CORHIO manages one of the country’s largest and most successful health information exchange (HIE) networks, provides advisory services that help healthcare professionals effectively use technology and improve care delivery, and supplies health plans and accountable care organizations (ACOs) with valuable data that enhance analytics and population health programs.
 

The Colorado Rural Health Center is the State Office of Rural Health and works with federal, state and local partners to offer services and resources to a constituency of over 3,500 rural healthcare providers, facilities and communities, and individuals.
